03 Alignment

Where Are You Right Now?

Choose the engagement designed for your current operational reality.

Immediate Phase

"I need to get my footing back."

The 30-Day Rock Bottom Reset

For someone who needs structure, clarity, and a defined framework to work through the immediate aftermath. When panic is high and you need daily parameters to stop making costly unforced errors.

  • You are dealing with an acute event or recent disruption.
  • You need immediate stabilization and personal routines.
  • You prefer private, self-paced work on your own schedule.
  • You need to triage the Five F's before committing to long-term planning.
Reconstruction Phase

"I'm ready to rebuild."

Rock Bottom Blueprint

For someone ready for a longer, structured process with accountability and a cohort environment. You have moved past acute panic and are focused on reconstructing a viable, durable professional future.

  • The initial acute shock has been steadied or managed.
  • You are ready for a substantive 12-week commitment.
  • You value high-trust peer counsel with other physicians.
  • You need a concrete blueprint for what your life and work look like next.
04 The Framework

Start with what you can control.

During a professional crisis, your instinct is often to fight on every front at once. The 30-Day Reset isolates five critical arenas and brings rigorous, daily discipline to each.

01 / PILLAR

Fear

Calming the acute panic response, neutralizing impulsive decisions, and regaining clear, unemotional judgment under pressure.

02 / PILLAR

Faith

Anchoring your foundational principles and personal integrity when your external identity as a practicing physician has been shaken.

03 / PILLAR

Family

Protecting your essential relationships, communicating with transparency, and preserving the home front when everything outside it is unstable.

04 / PILLAR

Fitness

Maintaining physiological discipline -- uncompromising sleep, nutrition, and physical exertion -- to ensure your body can withstand prolonged stress.

05 / PILLAR

Financial

Auditing cash reserves, rationalizing burn rate, and defending your balance sheet against immediate legal and professional exposures.

05 The Longer Rebuild
Surviving the moment is not the same as rebuilding after it.

The acute crisis passes, but what remains is the vacuum of what comes next. That cannot be addressed in a weekend, nor through solitary rumination.

Rock Bottom Blueprint is an intensive twelve-week cohort designed specifically for physicians who are prepared to do the systematic work of rebuilding. You will work alongside colleagues who understand the terrain, the losses, and the exacting standards of the profession.

Cohort Parameters
  • 01 12 Weeks of rigorous, sequential operational modules
  • 02 Structured Work with concrete deliverables each week
  • 03 Forward-Looking strategy for viable, sustainable reinvention
  • 04 Confidential Cohort of fellow physicians walking the same road
  • 05 Rebuilding Rather Than Surviving -- restoring long-term agency

Portrait of Dr. Jack Manzella, DO
Dr. Jack Manzella, DO The Rock Bottom Doc

Built from experience.

Dr. Jack Manzella trained and is board-certified in Internal Medicine and Pediatrics. His career included direct patient care, clinical research, and service on data safety monitoring boards.

Then he lost all of it. His career collapsed. His marriage ended. He lost his family, his medical license, and his freedom. He battled alcohol and addiction.

He served his sentence, completed parole, and committed to recovery. Then he did the demanding work of rebuilding, without a map, one day at a time.

He does not offer generic encouragement or theoretical counseling. Dr. Jack and his team provide structured, practical guidance to physicians navigating these treacherous waters toward recovery, both professional and personal. It is guidance built in the aftermath of actual catastrophe, by someone who has been to the bottom and climbed out.
